Lucent’s TPAs currently manage health benefits for over 250,000 members.
Information Needed:
- Broker’s Name
- Name of Brokerage Firm
- Broker Contact Information (Including phone, address, and email)
- Client Company’s Name
- Client Address (Including street, city/state/zip)
- Number of Employees
- Effective Date
- SIC (If available)
RFP Summary:
- Compensation Required
- Current and Proposed Benefit Plan
- Current Rates and Factors (including specific deductible and contract basis)
- Most Recent 24 Months Claims Experience (including monthly enrollments)
- Employee Census with Coverage Type (please provide electronically (DOB, gender, plan election, dependent status, zip codes, COBRA & retirees)
- Most Recent 12 Months Shock or Large Claim information (including diagnosis, prognosis and amount paid ytd)
- Stop-Loss Requirements for Specific Deductible
- Contract Basis for Specific Aggregate Coverages
