Plan members can use the MyLucentHealth member portal to view your ID card, SBCs, claims and coverage information, forms and documents, and more.

HSTConnect Portal

Members with an VBP plan that prefer to coordinate their care themselves instead of connecting with Concierge Care can use the HSTConnect mobile app or online member portal. HSTConnect allows members to find/compare providers or facilities, review and navigate balance bills, and more.

Members with an Anthem PPO plan that prefer to coordinate their care themselves instead of connecting with Concierge Care can use the link below to search for an Anthem provider in their area. VBP members can also use this link to find a facility or ancillary service.

If you have an upcoming procedure scheduled that was covered by your previous benefits plan, fill out the form below to notify Lucent Health’s Concierge Care team. Concierge Care will reach out to you to review your request for authorization of coverage under your new Lucent Health plan.
